David T. Applegate II, M.D.

David T Applegate II, M.D. graduated high
school in Detroit, Michigan and attended The Ohio State
University as a National Merit Scholar. Active in Helix
Biological Honor Society (Pres 1981), Ohio Staters, Inc. (Pres
1982), Ohio Unions Activities and many others, he was presented
a 1981 Outstanding Senior Award by the OSU Alumni Association.
He earned his BS in Biochemistry and continued graduate work in
Biochemistry. He graduated from The Ohio State University
College of Medicine in 1988 before completing a residency in
Family Medicine at Riverside Methodist Hospitals. He is Board
Certified in Family Medicine and is in private practice at COPC/
Marysville Primary Care. He is a Clinical Associate Professor,
Dept of Family Medicine, in the OSU College of Medicine. He is
on the medical staffs at Memorial Hospital of Union County,
Riverside Methodist Hospitals and Dublin Methodist Hospital. He
served as Chairman, Department of Medicine for Memorial Hospital
(1993-2001). After delivering more than 1600 babies, he retired
the obstetrical portion of his practice in 2001. Just prior in
2000, he was elected Coroner of Union County (Marysville, OH).
He currently is President of the Ohio State Coroners
Association and has served on their Board of Directors for the
last 5 years. He is a past president of the Charles B. Mills
Center Board of Directors (Mental Health facility). He
currently serves as Medical Director for the Union County
Sheriff’s Office and 6 Union County Fire Departments. He
currently serves on the boards for the Central Ohio Primary Care
Physicians (COPC) and the Medical Group of Ohio (MGO). He
enjoys volunteering as a Team Physician (football) and medical
advisor for the Marysville Exempted Village School District. He
belongs to the Ohio State University President’s Club. He is
active in his church as lector, Eucharistic minister, sacristan
and co-coordinator of altar servers. He serves in the
ecumenical Central Ohio Emmaus community as well. He is an
instrument-rated pilot and received the National Aeronautical
Association’s prestigious Volunteer of the Year Award in 2004
for flying blood from Oklahoma City to New York City on the
night of September 11, 2001. In 2004, his peers selected him as
OAFP’s Family Physician of the Year and since 2005 he is listed
in the prestigious “Best Doctors in America” register. He
resides with his lovely wife, Dr. Mary Applegate, and their 3
children in Marysville, OH.
